A child was observed walking into the office without supervision and caregivers were unaware he had walked away.. In another incident children were observed transitioning inside without proper supervision, as a caregiver was observed walking to get a child who had walked off, leaving the children in the middle of the line were unsupervised. The children in the end of the line were supervised by a second caregiver, however she could not see the children in the middle of the line.
