The outdoor area had a flat table device with water in it, planting soil was left out and accessible, and the water sprinkler head was lying in the grass were children were running, and it could have caused a tripping hazard. The rug in the Preschool room was rolled up on the ends, and was a tripping hazard. Corrected at inspection. All items were moved from the playground and the rug was removed from the room.
