The school-age outdoor area had a pile of stacked wood by the fence that could cause an unsafe environment; The wooden bridge had uneven planks and a loose plank that caused an unsafe environment; the tree branches were hanging on top of the playground and children were swinging on them and climbing the branches and it caused a supervision issue in another area of the playground.
