      Safety - Areas Free From Hazards

      After being aware of a child who was able to open the playground gate with ease on more than one occasion, immediate action was not taken to better secure the gate. Video of an incident was observed where the child opened the gate and ran before being retrieved by the caregiver on 3/25/26. At the time of the investigation inspection on 4/2/26, it was observed that a correction to better secure the gate had not yet occurred, which showed continued risk.
