On the wooden fencing surrounding the outdoor play area, there were: two spots where nails were exposed at the bottom of the fence; two spots where the slats had gaps at the bottom of the fence; and one section of fencing that had pulled away from part of the fence which caused a gap in the fencing at the children's level.
