Based on information gathered during this investigation, staff failed to count the children they were responsible for resulting in a child being able to exit the operation unsupervised. Staff was unaware the child was missing for approximately 15-20 minutes until they were contacted by law enforcement.

Based on information gathered during this investigation, staff did not demonstrate good judgement because caregivers were aware of a child's tendency to elope and required increased supervision. Despite the known behavior, staff did not take the needed precautions to ensure the child's safety and measures were not implemented to prevent the child from exiting the gate.

Based on information gathered during this investigation, a child was able to exit the operation without supervision. The child walked down the street and was eventually taken to the police department by a passerby. The child was gone for an extended period of time.
