Two staff did not conduct a name to face while lining the children up to go outside while in the room before opening the door and when outside on the porch area to ensure all children were accounted for before going out on the playground. During the course of the investigation, it was determined that all bottles and cups were properly labeled. However, a caregiver failed to ensure that the correct bottle was given to the right child, resulting in an infant being fed another child's mother's breastmilk.
