    This standard was reviewed as part of an investigation and found deficient. A caregiver failed to identify and remove hazards in a classroom by placing a glass jar on top of an unstable storage bin that was accessible to children. This resulted in a child being injured and obtaining medical attention. The glass jar and storage bin have been removed.
