Based on the information obtained during the DFPS investigation, it was determined that a caregiver tried to force a child to sleep by roughly handling the child then grabbing the child?s arm and holding on to prevent the child from moving off the nap cot.

Maintenance of Building, Grounds and Equipment
The pool noodles covering the poles of the shade structure in the outdoor playground were not maintained. The pool noodle material was deteriorating and missing pieces. This posed a safety hazard for children as the pool noodles were accessible to children and smaller children may accidentally ingest small pieces.
