An infant was asleep in an infant carrier/stroller for more than 5 minutes and was not immediately removed and placed in the pack n play. The provider placed the young infant in a pack n play before the inspection was concluded. Infants sleeping in restrictive devices are at risk for strangulation, injury, and positional asphyxiation.
