It was determined during the investigation that a caregiver did not demonstrate competency. The caregiver knew there was rule in place by the YMCA to not allow children to use the monkey bars; however, the caregiver allowed multiple children to use the monkey bars without stopping them, and it led to an injury to a child later in the day.
