This standard was evaluated as part of the investigation and was found to be deficient. Through interviews of staff, the victim, and a collateral adult, and through review of video, it was determined a caregiver was not properly supervising the children in care when one young child left the classroom without their knowledge and exited the building. The young child was returned to the operation by an adult not associated with the operation. The young child was unsupervised by the operation for 5 minutes and 46 seconds.

Children were observed napping on mats not labeled with a child's name or with a number and have a map listing the name of the child that is associate with the number. This will help reduce the spread of germ. The caregivers sanitized the mats after nap time, and they will label the mats moving forward.
