Multiple vents in the restrooms and throughout the facility were observed covered in dust, dirt and other particles. Ceiling tiles throughout the facility were observed with water stains. A ceiling tile in the pre-k room was observed brown in color and bulging. The window seals in the pre-k room were observed with dust, dirt and debris. The outdoor area was obserevd with lifted or torn pieces of outdoor carpet and rusted staples.

The infant room was observed with a broken acrylic lighting panel. The wooden fence was observed with a broken panels and 2 wooden studs. Four tricycles were observed with a missing handle grip cover which pose a risk for the children putting their finger inside the handlebar gap during use. The boy's restroom was observed with a broken hinge which makes the door not close correctly.
