During the investigation it was determined that children have been injured when caregivers did not intervene in the activity that caused the injury. Groups of children were also observed, during inspection, to be running through the building, from one class area to another without structure of the caregiver being able to see where the children are or what they are doing. The management of children's behaviors are not under control.
