Four staff records were reviewed: All staff need the following: Emergency Preparedness Medication Building and Premises Safety Communicable diseases Food allergies Hazardous Materials CG and J need SIDS/Shaken Baby/Abusive Head Trauma CG needs 5 hours more child development and 2 business management Judy needs 12 hours of child development - the required training above can count towards these hours.

During the inspection I observed drywall falling out and paint chipping in the after school bathroom, as well as this I observed old food particles and dead bugs on the floor of multiple rooms.
