I evaluated the enrollment record for a pre-school child who is in care and observed that the enrollment record was missing the following information: 1. Parent authorization for emergency medical care. 2. Phone numbers for persons authorized to pick up the child from the child care home. 3. Information indicating whether or not the child has any special needs. 4. An updated immunization record.
