      Licensing Incident/Illness Report Form - Situations Placing Children at Risk

      During the investigation it was discovered that the operation failed to accurately document all known information regarding a situation that placed a child at risk. After management reviewed video footage and identified additional events that may have contributed to the child?s injury, the incident report did not initially reflect those findings and required revision. Therefore, the Licensing Incident/Illness Report Form was incomplete and did not accurately document the circumstances known to the operation.

      During the investigation it was learned that the operation failed to ensure the parent was timely informed of all known information regarding a situation that placed a child at risk.
