      Records were evaluated for ten children enrolled and the following was determined: Two children needed emergency contact information. Three children needed permissions for transportation. Three children needed permissions for field trips. Four children needed permissions for water activities, including if whether the child is able to swim without assistance. One child needed an updated immunization record. Incident reports completed for two children in care were missing information such as the date and parent/director signature.

      Based on the information obtained, it was determined a caregiver did not maintain proper supervision while watching two separate groups of children ages three and four years old. The caregiver left one group unattended multiple times to attend to the classroom and retrieve an item from the classroom.
