Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity
The operation failed to ensure that a caregiver adequately supervised a child at all times. During outdoor play, the child left the playground through the gate, entered the building, to change his clothes without the caregiver responsible for his supervision's knowledge. The caregiver was unaware the child had left the playground until another staff member located the child inside the building and notified her. The child was able to leave the designated play area and remain unsupervised, demonstrating that appropriate supervision was not maintained.
