In the restroom that the children use, there was cleaning chemicals on top of the sink and in the unlocked bottom cabinet. There was a basket on the floor, next to the sink with diaper cream. The care giver's purse was within reach of children. During the inspection, the director removed the cleaning chemicals, the basket with diaper cream from the restroom. and the care giver's purse away from the children.

Itt was observed that the fence in the preschool outdoor play area has broken wood boards with holes and gaps at the bottom of the fence of 7 inches. The infant and toddler outdoor play area has gaps under the fence causing large holes due to children digging up the soil. The art easel in the Tulip Patch classroom had a broken tray that was missing a screw causing the tray to be unsecured.
