During the visit, a six-month-old slept in a restrictive device. The infant was not strapped correctly in the restrictive device. The infant woke up in the restrictive device when the inspector asked the caregiver to remove the blanket from the infant. The caregiver removed the infant from the restrictive device during the visit.
