The back outdoor playground area contained hazards such as 2 feet of rebar sticking out of the ground near the swings. The sidewalk is a tripping hazard due to uneven, broken concrete. There are various broken toys, furniture and trash on the back outdoor playground. The surfacing is not maintained under the swings. Lastly in the front play area accessed by the children there is a hole in the side of the building where a stray cat gained access.
