A caregiver used poor judgment when she tied a piece of fabric around a small child's eyes in an attempt to help the child sleep. The caregiver created unsafe sleeping conditions as the child was also lying on their stomach with a pacifier in their mouth. The caregiver also positioned nap mats, draped with a blanket, around a young child during nap time.

On separate occasions, a caregiver placed a weighted blanket and a weighted pillow on a small child's body during nap time. In addition, the same caregiver tied fabric around a child's head, covering their eyes to encourage the child to sleep.
