Safety Requirements for Cribs - Snug Fitting, Firm, Flat Mattress Designed for Use in Crib
Sheets did not fit snugly in cribs and were bunched around the sleeping baby. There was space between the mattress and the crib base that was accessible to an infant. The crib mattress did not fit tightly in the crib. Corners of the mattress were curling up. A pacifier cord was attached to the baby while sleeping in the crib. The cord was removed at inspection.
