Based on the evidence obtained during the investigation, the staff were unaware of the on-going activities and lacked auditory and visual awareness of a child they were responsible for when the child left the classroom to the outdoor space area. The door closed behind the child after entering the outdoor space. Additionally, the child was discovered by another staff member whom made the caregivers responsible for this child, aware that the child was missing.
