It was determined that at least two children were unsupervised in an empty classroom pushing on a door. While this was occurring, a third child is seen pulling on the door and staff were not aware of what was happening.

Maintenance of Building, Grounds and Equipment
It was determined that the Dutch door in between the classrooms had a broken latch that prevented both halves of the door to be latched. As a result, both halves of the door could swing independently of each other causing a safety concern.

Caring for Children Above or Below Ground Level-Written Approval
During the inspection at the program children were present and engaging in activites on the second level of the facility without written fire marshal approval. The director did relocate the children to another area of the facility during the inspection and the children will no longer be present on the second level until the fire marshal gives the approval.
